Mount Lebanon accepts Milwaukee and Wisconsin Voucher students as well as traditional tuition paying families.

Enrollment Information

  1. We are here to assist you with enrollment by phone (414)464-5410 or in person at 6100 W. Hampton.

  2. The income limits for a family to participate in the Milwaukee Parental Choice Program (MPCP) and the Wisconsin Parental Choice Program (WPCP) can be found at the DPI Website.

  3. If you do not qualify for the MPCP or the WPCP, please contact the School Office at 414-464-5410 to learn more about our traditional tuition option.

  4. Open enrollment for the MPCP is typically the 1st-20th of each month beginning in February. Open enrollment for the WPCP is February 1st to April 2oth.

Steps to Enroll Online

1) Complete DPI Requirements Online: https://apps4.dpi.wi.gov/ChoiceParent/

2) Bring the following items to Mount Lebanon Lutheran School

a. Proof of Address:

Current wage statement

Utility bill

Lease agreement

Tax bill

Government issued letter

B. Verification of Income:

Last year's taxes if filed

Your W-2 Form (Wage and Tax Statement)

If you have no taxable income and receive government subsidies, bring:

Food Share information

Supplemental Security Income (SSI) proof

Wisconsin Works Cash Benefits (W2) statement

Temporary Assistance for Needy Families (TANF)

Rent Assistance document

A student in the Foster Care/Kinship program needs to provide

proof of participation – no other income verification is required.
